Individual Health Record - IHR

IHR – Individual Health Record

The Individual Health Record is an electronic system used by the Out of Hours teams and hospitals to access your medical information when the surgery is closed.

A summary of your medical notes is taken from our clinical system and can only be accessed with explicit consent. This can be very useful to the consultants or emergency teams when managing your health to help find out significant medical history or your allergies.

Should you be unconscious for any reason and a next of kin does not know your medical history, the emergency teams can access your details to help ensure you are treated correctly.

A thorough audit trail is maintained and only those with a reason for access will view your record.

If you wish to opt out of this service please contact the surgery for further information.
